An official with the Transportation Safety Board says a complex series of events led to the crash of a water bomber near La Ronge last year that killed a local man.

54-year-old Andy Clark died after a Convair 580A he was serving on crashed at the La Ronge airport on May 14th, 2006.

A report by the TSB cites poor communication between pilots, inconsistent training documents and pilot confusion as factors contributing to the tragedy.

TSB regional manager Peter Hildebrandt says the plane was approaching the runway for a landing when it slowed down to a below-normal speed.

Hildebrandt says the crew increased the power to the engines, but one of them shut down.

He notes the crew was apparently unaware of this when they tried to land.

Hildebrandt says more staff have been hired at the northern air operations office as a result of the crash.

As well, a full copy of the report has been sent to Convair and to aviation officials.
